| Patient services is provided by The Head+Neck Center to help make your visit to our office a pleasurable experience. This section enhances the patient-physician relationship by giving you 24-hour access to patient specific instructions and communication tools. |

New Patients |
| We are always happy to accept new patients. Please contact our office to schedule an initial consultation. Patients can save time in the office by printing our online forms (see list to the right) and filling them out at home, as well as reviewing pre- and post-operative instructions (see list to the right). |
Patient Forms |
| All new patients need to complete the New Patient Information and Patient Medical History forms online. They will be securely transmitted to our office upon completion. |
| If any patient information has changed since your last visit, such as your insurance carrier or your address, please complete a new Patient Medical History form. Please bring your insurance card and photo identification to all appointments, as well as a referral from your primary care provider, if required by your insurance company. |
Patient Procedures and Instructions |
| We offer downloadable instructions for pre- and post-operative care (see list to the right). The doctor will review these instructions with you before your procedure. If you have any questions, please discuss them with our office before your procedure. |
